have you opened up the case and made sure the antenna wires are securely plugged into the card? I doubt they would pop off on their own, just a thought. Maybe it's going haywire and needs to be replaced. Try reseating the card in it's...
try uninstalling it from Device Mangler and let it rediscover it. Also try the latest drivers if you can find anything newer.
