Largest game worlds I believe would be The Elder Scrolls 2: Daggerfall. The map was around 487,000 square kilometers. There were roughly 15,000 locations to visit in the game and roughly 750,000 NPC's.
That blob of flesh and gore WAS Princess. What happened was she (somehow) took enough damage that she should have insta-gib'd. But the games engine is programed to keep kids alive no matter what to try to avoid any criticism from a game...

As long as Broken Steel is installed and active, you can still access any missed quest.
Oh gawd, my adventures with the gamebro engine between Oblivion, Fallout 3, and New Vegas...........I've never seen so many funny glitches in an engine before
