I had this problem also. I am using raid. I was using a cheap usb floppy from CompUSA. I tried a friends IBM usb floppy and it worked without a hitch, I think the bios has a problem recognizing some floppies. When I first go the machine and plug in my floppy it saw it as a 5 1/4 floppy!!!
I ordered a usb floppy from pctorque a and I used that. It worked without any problems. I also asked pctorque to put up instructions/note somewhere saying floppy is recommended or something like that.
This works with windows 2000, XP and windows 2003 installs
