Graphics Card Overheating

So I have an hp dv5t notebook, and i bought it new about 2 years ago. Originally, it was fine, but over the past 6 months or so, it has been seriously overheating. My graphics card idles at 60 degrees Celsius, and as soon as i do anything that puts any kind of strain on it, it shoots up to around 90-95. I know this is seriously wrong, but is there anything i can do about it? Or is it just an old laptop thats reached the end of its life time?

2 years is a long time. Time to take it apart, cleaning it up and put in new thermal paste.

Looking at the heatsink and fan assembly, it would be well worth stripping it out and separating the haetsink from the fan and giving the fins on the heatsink a good clean...
