What causes dead pixels?

Is it users' fault, or is it the manufacturer's?
Is there any way to prevent em?

Technically, it isn't anyone's fault... it's just the limit of our LCD production technology. It's impossible to continuously produce 100% defect free LCD panels with our current technology... so every now and then, a pixel has problems.

I hear that extreme temperatures may cause dead pixels sometimes...

Let's see, my screen is 1280 by 800 by three primary colours... That's 3,072,000 transistors in 2681 square inches, or about 1145 transistors per square inch. And unlike RAM or Processor transistors, bad ones can not be bypassed. I'm awed that they can make any that have zero defects... (I have one bad Pixel, came that way, doesn't bother me)...
